My COVID-19 Vaccination Documentation - External Users
Please DO NOT use this form unless you have been instructed to do so by Human Resources. Caltech community members should refer to the Caltech COVID-19 Reporting Web Application to submit their documentation.
NOTE: If you have received a multi-dose vaccine (Pfizer, Moderna), do not upload your documentation until you have received your second dose and your vaccination record is complete.
- Your vaccination card or immunization record image, must include your full name, DOB, vaccine name (Pfizer, Moderna, Johnson & Johnson), and date(s) of vaccination.
- To upload your documentation, click Select Files
- Select the file you want to upload, the name of the file will appear on the Upload files window
- Click Submit
- Go to STEP 2 on the right side of this page
- Make sure to upload your vaccination documentation image as part of STEP 1.
- An email address or phone number is required to receive your confirmation and QR code.
